New Patient Registration

Featherstone2You must live within the surgery boundaries to be able to register as a patient.  Please speak to a member of the reception team who will advise you.

The surgery has a registration policy and this includes various information that we will obtain from you before you register as a patient. All information given is governed under the Data Protection Act and all staff have signed a confidentiality agreement.

All relevant registration forms are available at the surgery, please call in to collect.

Medical treatment is available from the date of registration. Please contact reception for further information.
